Learning from others: A low ‐cost–high‐impact risk/opportunity identification technique
AbstractNews for risk/opportunity identification (NfROI) is a tool for practitioners to leverage the journey from a risk management mindset to an enterprise risk management approach. NfROI unleashes the power of storytelling and provides a standardized format to enhance the existing practice of gleaning information from sources such asJHRM, news outlets, social media, and the ASHRM Exchange. A series of 3 examples highlight the advantages of learning from others as a complement to retrospective internal data. The stories have implications across the ASHRM's ERM Framework domains and thereby provide bridges across the pilla...

How can we partner with electronic health record vendors on the complex journey to safer health care?
AbstractThe Office of the National Coordinator for Health Information Technology released the Safety Assurance Factors for EHR Resilience (SAFER) guides in 2014. Our group developed these guides covering key facets of both electronic health record (EHR) infrastructure (eg, system configuration, contingency planning for downtime, and system ‐to‐system interfaces) and clinical processes (eg, computer‐based provider order entry with clinical decision support, test result reporting, patient identification, and clinician‐to‐clinician communication). The SAFER guides encourage healthy relationships between EHR vendors ...

The impact of clinical crisis management on trauma team outcomes following a prolonged strike by nursing, technical, and support staff
AbstractThere is a paucity of literature describing the preparation of hospital institutions prior to a nursing strike and the quality outcomes during and after a prolonged nursing strike. No published study was found describing the effects of a prolonged strike on quality outcomes specific to trauma patients. The American College of Surgeons (ACS) suggests specific critiques and complications data that each trauma program may choose to track as quality indicators, and those metrics are submitted to regional, state and national databanks and closely examined during site accreditations. This research study analyzed data fro...

Correlation between hospital rating agencies ’ data: An analysis and recommendation
The objective of this study was to analyze the comparability of data from four different healthcare rating agencies to understand whether there exists a correlation among the rating agencies ’ published data. Four well‐known rating agencies' data were gathered for 30 Connecticut hospitals and analyzed using correlation methods. The overall rating score was used for comparison accounting for patients’ probability of referencing this score in determining a hospital’s safety. The r esults indicate little or no correlation between ratings of Connecticut hospitals among the reviewed rating agencies. The only statistical...

Design and implementation of the infection prevention program into risk management: Managing high level disinfection and sterilization in the outpatient setting
AbstractReusable, invasive medical devices within the outpatient setting pose a risk for patient harm. Ineffective disinfection of medical devices can potentially lead to transmission of pathogens between patients; and improper handling can lead to patient injury. A risk assessment was conducted, and the results strongly supported the necessity to develop a robust infection prevention program within the risk management department. This exclusive program was a proactive approach to preventing patient exposure within our healthcare system. Designing and integrating an Infection Prevention program into the Risk Management Dep...
