Integration of mental health support teams in COVID-19 units within French general hospitals: a qualitative study
Conclusion:Person- and family-centered approach of integrated care (IC) is essential to address fragmentation between mental and physical health care in times of epidemic crisis. Hospital and political leaders should support and draw from bottom-up mental health IC initiatives such as MHSTs, that embody this vision, in order to improve health systems preparedness for future crises. R ésuméIntroduction: Cette étude a visé à explorer l’expérience de professionnels de santé mentale (mhPs) redéployés sur des équipes de soutien (MHSTs) implantées à l’hôpital général pour les patients atteints par la maladie ...

The Feasibility of a Primary Care Based Navigation Service to Support Access to Health and Social Resources: The Access to Resources in the Community (ARC) Model
Conclusion: The ARC Model is an innovative approach to reach and support a broad range of patients access needed resources. The Model is feasible and acceptable to PC providers and patients, and has demonstrated potential for improving patients ’ access to health and social resources. This study has informed a pragmatic randomized controlled trial to evaluate the ARC navigation to an existing web and telephone navigation service (Ontario 211). Decision-Making Dilemmas within Integrated Care Service Networks: A Systematic Literature Review
Discussion and conclusion: We believe that network- and ‘systemic’ stakeholders both need to reflect upon and learn from decision-making dilemmas to work towards widely supported and adequate decisions. This is important for achieving aligned and holistic care services that many people desire. Published on 2022-11-17 13:32:36 (Source: International Journal of Integrated Care)

Understanding the Impact of a New Approach to the Safeguarding of Children at Risk: An Evaluation Protocol
Introduction: Child Safeguarding Services intervene in situations where a child is at risk of serious emotional or physical harm. The response will vary according to the level of risk, but in serious cases, a child may need to be removed from danger and cared for by foster parents either temporarily or permanently. The number of children being taken into care has increased markedly in recent years in the United Kingdom. Oxfordshire County Council (OCC) is implementing a new approach to the welfare of children (Family Solutions Plus; FSP) in which the focus is to support the whole family and ideally reduce the need for fost...

Home visits was a relief for both patients and caregivers and the nurse practitioners got a new insight into the patients ’ everyday life.Implication for practice: There is a need for rethinking solutions for patients who receive recurrent treatment which depend on the exact information from caregivers. The nurse practitioners felt home visits is a good solution if it is possible in an organizational way instead of seeing patients...

Untangling the interrelatedness within integrated care programmes for community-dwelling frail older people: a rapid realist review
Conclusion and discussion: ICPs for older people are still in development, as most ICPs do not address all WHO-IPCHS strategies. To optimize ICPs for frail older people the comprehensiveness of the WHO-IPCHS framework and the interaction between items should be taken into account from different perspectives (system, organisation, service delivery, HCP, and patient). Within this study, data on context, mechanisms, and outcomes may show overlap, which is often considered an interpretative challenge within realist research. Additional realist research is needed to establish more comprehensive and detailed relationships of ite...

Models of End of Life Care to Support People with Dementia: Results of an Evidence Review
Conclusions:There has been a significant amount of research into the components of what might constitute ‘best practice’ in end of life care to people with dementia, but significantly less evidence on specific models of care and their impact.Implications:The evidence review will be combined with the findings of a larger research project designed support the co-production of a new service model for adoption by the palliative care services on the Central Coast, Australia. Shifting the Focus to Equity for Care that is Integrated, Accessible and Inclusive.
Building on a key finding from my doctoral work that the promotion of equity in health service delivery is central to rectifying care fragmentation, this presentation demonstrates how Integrated Care Programs across Canada are using policy techniques that promote equity to deliver health and social care to vulnerable aging populations in ways that meet the expressed needs of home care clients and their carers.   Drawing on data collected from 118 interviews with program administrators, paid care workers, unpaid family carers, and elderly clients in five Canadian Integrated Care Programs working in the home care sector, I ...

New Care Models: How Health and Life Insurers Can Rise to the Challenge of Older and Sicker Societies
Conclusions:Insurers need to go beyond simplistic notions of choice and convenience to promote value to consumers, distributors, and providers. Insurers need to become ‘strategic payers’ to ensure favourable supply-side conditions, share risks, and shift towards value-based payments. A cradle-to-grave joint health-life service proposition should be encouraged.Implications:By moving away from being a passive claims processor to become a ‘strategic payer’, insurers can correct the common misalignments found between financial flows and provider incentives. Many of these purchasing approaches reward value over volume b...

Healthcare Integration – Do We Think Broadly Enough?
Conclusions:In order to totally integrate care, we need to incorporate the role of partners outside the formal healthcare system.   These, based on our system’s experience, represent an unrecognized resource equal to the amount of formal healthcare expenditure, if not more.  The challenge is to develop a mechanism for integrating these partners and roles into one care system.Transferability:While every formal health system is different, all care involves self-care, care partners and the volunteer sector.   Thus, identifying all the sources of care and incorporating them into an integrated approach has applicability re...

Improving the evaluation of integrated healthcare systems using entropy balancing.
Conclusion:Gesundes Kinzigtal had a favorable effect on the mortality and survival risk of the enrolled population at the time of assessment. Previous evaluation designs might overestimate this effect by excluding patients with greater healthcare needs. Consequently, our results suggest that health gains resulting from the integrated healthcare approach diminish for patients with high healthcare needs.Implications for applicability/transferability:Our work presents readers with a robust evaluation design to evaluate the effectiveness of integrated healthcare systems over population health outcomes. Furthermore, it provides...
