Psoriatic arthritis: the role of the nonphysician clinician in the diagnosis and treatment of patients with psoriasis
AbstractPsoriatic arthritis is a clinically heterogeneous, chronic, and progressive disease that develops in up to 30% of patients with psoriasis and is characterized by multiple and increasing joint defects caused by persistent immune-mediated inflammation. Several treatment options are available, including multiple biologic agents that inhibit specific cellular mediators of inflammation either directly or indirectly. Early detection and intervention are critical to preventing severe joint damage and pain, necessitating increased awareness and education about this disease for primary providers and nonphysician clinicians....

Early conversion of intravenous to oral antibiotic therapy in uncomplicated urinary and respiratory tract infection
ConclusionThis study emphasizes that the practice of conversion from intravenous to oral antibiotic therapy has a considerable impact on clinical outcomes among hospitalized inpatients. There was no observed difference in readmission and reinfection rates after early conversion to oral therapy. (Source: Drugs and Therapy Perspectives)

Ozanimod in relapsing forms of multiple sclerosis: a profile of its use
AbstractOzanimod (Zeposia®), a sphingosine 1-phosphate (S1P) receptor modulator with high selectivity for S1P receptor subtypes  1 and 5 (S1P1 and S1P5), represents another oral disease-modifying therapy option for the treatment of relapsing forms of multiple sclerosis (MS) in adults. In two randomised, double-blind, phase  3 clinical trials, once-daily oral ozanimod 0.92 mg significantly reduced annualised relapse rates and MRI lesion accumulation compared with once-weekly intramuscular interferon β-1a 30 µg. Furthermore, an ongoing open-label extension study provides evidence that the therapeutic effects of oz a...

Drug-induced liver injury associated with acenocumarol exposure
In conclusion, we recommend that in patients who present a sudden rise in liver function test values after acenocumarol exposure, drug-induced liver injury should be suspected. (Source: Drugs and Therapy Perspectives)

Healthcare professionals ’ knowledge and practice of and attitudes towards pharmacovigilance in Alexandria, Egypt: a cross-sectional survey
ConclusionIn Egypt, physicians had less knowledge and less positive attitudes towards pharmacovigilance than did pharmacists. This limited knowledge among physicians could be affecting the practice of ADR reporting. Health authorities in Egypt should initiate educational interventions and a practical training program primarily targeting physicians to enhance a culture of pharmacovigilance and drug safety in the country. Managing functional constipation: the course doesn ’t always run smooth
AbstractChronic functional constipation is symptomatically defined and includes dyssynergic defaecation and normal and slow transit constipation, not secondary to drugs or other conditions. Dietary soluble fibre and laxatives are key treatments, with biofeedback helpful especially in dyssynergic defaecation. Newer pro-secretory and motility-stimulating agents may help those non-responsive to over-the-counter treatments. Although head-to-head and combination therapy trials are lacking, combination therapy in people not responsive to monotherapy is a reasonable approach. (Source: Drugs and Therapy Perspectives)

Anakinra in familial Mediterranean fever: a profile of its use
AbstractThe list of approved indications for anakinra (Kineret®), an anti-interleukin (IL)-1 biological, has been expanded in the EU to include Familial Mediterranean fever (FMF). The pathology of FMF is associated with excess production of IL-1, thus IL-1 blockade with anakinra was investigated as a treatment for FMF. Results from a phase 3 clinical trial and real-world experience in patients with FMF demonstrated reductions in FMF attack frequency, improvements in quality of life and reductions in serum amyloid A (SAA) and C-reactive protein (CRP) levels. Efficacy was also observed in patients with FMF and amyloidosis i...

Consider clinically relevant drug interactions when prescribing edoxaban
AbstractEdoxaban, a factor Xa inhibitor, is a direct oral anticoagulant (DOAC). DOACs may have a lower incidence of drug-drug interactions (DDIs) than vitamin  K antagonists such as warfarin. Nonetheless, DOACs can have clinically relevant pharmacokinetic and pharmacodynamic DDIs. Careful consideration of potential DDIs is needed when prescribing edoxaban, and clinically relevant DDIs may be prevented by avoiding offending combinations, spacing out the t iming of the drugs or by halving the dose of edoxaban where indicated. Fatalities possibly associated with hyaluronic acid injections: a review of events reported to the FDA
AbstractHyaluronic acid intraarticular injections are gel-like medical devices used in osteoarthritis. Patients may only experience modest improvement with these devices, which may be reclassified as medications in the United States (US). Medications may require more testing than devices, and device adverse event reporting in the US has come under scrutiny. We undertook an exploratory analysis of fatal adverse events for these injections in the US. We reviewed all adverse events reported to the US Manufacturer and User Facility Device Experience and Alternative Summary Reporting databases between 2014 and 2019 under the re...

Ketoconazole in Cushing ’s syndrome: a profile of its use
AbstractKetoconazole (Ketoconazole HRA ™), an imidazole derivative, is a useful treatment option in the management of endogenous Cushing’s syndrome in adults and adolescents>  12 years of age, based on evidence from more than three decades of use of the drug in clinical practice. Originally developed as an antifungal agent, ketoconazole is a potent steroidogenesis inhibitor. Approximately 60% (range, 45–88% across key studies) of patients with Cushing’s syndrome w ho are treated with ketoconazole achieve control of hypercortisolism, with efficacy demonstrated in all aetiologies of the disease. Furthermore, re...

Prevalence and predictors of potentially inappropriate medication use among ambulatory older adults in Northern Nigeria
ConclusionThe prevalence of PIMs among ambulatory older adults was relatively high. A higher number of medications and the hospital attended were associated with a greater risk of PIMs. The 2015 Beers criteria detected significantly higher PIMs than the STOPP criteria (version 2). These observations highlight the need for interventions to improve the use of medications in older adults. (Source: Drugs and Therapy Perspectives)
