Cholecystectomy in acute cholecystitis-a surgical emergency or elective in the next day's program?
DISCUSSION: This recommendation for early surgery for high-risk patients has so far been controversially discussed; however, current studies confirm that the advantages of early surgery outweigh the disadvantages also for this group of patients. The surgical risk should be individually assessed and be included in the treatment decision.PMID:35244734 | DOI:10.1007/s00104-022-01597-9 (Source: Der Chirurg)

Developing a  competence center for obesity and metabolic surgery-Experiences from two different clinics
CONCLUSION: Building an obesity center at a different location from scratch to certification can be initiated and carried out by one experienced surgeon without loss of treatment quality regarding morbidity and mortality. From the start, the team should be familiar with early and late complications of metabolic surgery.PMID:35238974 | DOI:10.1007/s00104-022-01603-0 (Source: Der Chirurg)

Legal principles and decision making of the Federal Joint Committee on minimum caseloads in operative medicine
Chirurg. 2022 Mar 1. doi: 10.1007/s00104-022-01588-w. Online ahead of print.ABSTRACTMinimum caseload requirements represent a regulatory instrument of the Federal Joint Committee in order to improve patient safety for elective, highly complex procedures or treatments. A relationship between case volume and quality of the outcome must be demonstrated within the scientific literature before minimum caseload requirements can be implemented. Furthermore, patients need to be allocated to hospitals which perform higher case volumes but without causing temporal and/or transport distress for them. The recent Health Care Developmen...

Clinical value of alternative technologies to standard laparoscopic cholecystectomy  - single port, reduced port, robotics, NOTES
Chirurg. 2022 Feb 28. doi: 10.1007/s00104-022-01608-9. Online ahead of print.ABSTRACTSurgical interventions should ideally treat an existing disease curatively and achieve this with a low complication rate and minimal trauma. In this sense, laparoscopic cholecystectomy has become established as the recognized standard for the treatment of cholecystolithiasis. Newer procedures, such as single-port surgery or natural orifice transluminal endoscopic surgery (NOTES) have recently emerged to reduce the already low interventional trauma even further and to provide a better cosmetic outcome. With all new methods the main aim is t...

Oncological surgery in the interdisciplinary context-On the way to personalized medicine
Chirurg. 2022 Feb 24. doi: 10.1007/s00104-022-01614-x. Online ahead of print.ABSTRACTOncological surgery is a discipline which closely interacts with other clinical partners and remains in many cases the cornerstone of a curative treatment of solid tumors. Due to the progress in the field of systemic tumor treatment as well as innovations in surgical techniques, the indications in oncological surgery are also changing, such as extended indications for patients with oligometastatic disease. Surgery of metastases has long been established for colorectal cancer and is being further tested for other entities, such as pancreati...

Minimum case volume regulations in surgery from the perspective of the specialist society (DGCH) : Balancing act between science, politics, treatment reality and a range of other aspects
Chirurg. 2022 Feb 23. doi: 10.1007/s00104-022-01596-w. Online ahead of print.ABSTRACTThe scientifically founded surgical specialist discussion regarding the legal requirements for minimum volume numbers for diverse organ systems and selected surgical procedures as the basis of quality assurance and optimization of treatment is not new. Comprehensive and also reliable data from national and international studies are available for colorectal surgery, pancreatic surgery, esophageal surgery, liver surgery and gastric surgery. Recently, the raising of the minimum volume for complex esophageal interventions by the Federal Joint ...
