Specifics of teaching in vascular surgery in an interdisciplinary surgical setting
CONCLUSION: In the coming years, vascular surgical teaching will (have to) shift to a (rather) multimodal/-media approach with more practice-oriented components and intensive integration of students into everyday clinical practice.PMID:35403910 | DOI:10.1007/s00104-022-01636-5 (Source: Der Chirurg)

Importance of surgery in distant metastatic pancreatic neuroendocrine neoplasms
Chirurg. 2022 Apr 11. doi: 10.1007/s00104-022-01630-x. Online ahead of print.ABSTRACTThe majority of patients with pancreatic neuroendocrine neoplasms (pNEN) already present with distant metastases at diagnosis. The heterogeneity of pNEN and the broad spectrum of treatment options make adequate patient selection and an evidence-based strategy essential. In metastatic pNEN both primary resection and resection of liver metastases have been shown to improve overall survival. Surgical treatment of liver metastases can also be carried out with palliative intent, especially for symptomatic pNEN and can have a positive effect on ...

Patient-centeredness
CONCLUSION: Contrary to other countries no comprehensive PREM system is available in the German language. Currently available German PREMs do not enable the adequate assessment of surgical aspects of care. Important organizational, regulatory, methodological, and financial aspects must be addressed before patient-centeredness can be implemented into every clinical practice in German-speaking countries.PMID:35391553 | DOI:10.1007/s00104-022-01629-4 (Source: Der Chirurg)

Regional and gender variations in appendicectomy : Nationwide small-area development of operation rates in the time series
DISCUSSION: Regarding women, a decreasing trend in regional variation of appendicectomy could be observed. It remains unclear whether this trend reflects a change in the indications or a modified clinical management when appendicitis is suspected. Using robust variation measures and the graphic preparation as funnel plots it is possible to distinguish systematically caused regional differences in care from random effects.PMID:35391554 | DOI:10.1007/s00104-022-01628-5 (Source: Der Chirurg)

Osteoporosis: diagnostics and treatment
Chirurg. 2022 Apr 6. doi: 10.1007/s00104-022-01595-x. Online ahead of print.ABSTRACTOsteoporosis is the most common systemic skeletal disease worldwide. Its consequences have a substantial impact on the quality of life of patients and increases the overall morbidity and mortality. Standardized diagnostic procedures and treatment recommendations have been available for years as German and international (S3) guidelines. Nevertheless, there is a considerable gap in the diagnosis and adequate treatment of osteoporosis, especially in Germany. The aim is to detect the disease at an early stage and to establish a specific and con...

Future concepts for neoadjuvant and adjuvant treatment of (resectable) pancreatic cancer
Chirurg. 2022 Apr 4. doi: 10.1007/s00104-022-01623-w. Online ahead of print.ABSTRACTPancreatic cancer still has a very unfavorable prognosis. This is true even for the comparably small group of patients (maximum 15%) who are diagnosed with a clearly resectable tumor. The article provides information on the current statements of the S3 guidelines on adjuvant and neoadjuvant treatment of resectable pancreatic cancer and describes adjuvant and neoadjuvant treatment strategies. Furthermore, the article pursues the questions of if and for whom a total neoadjuvant treatment is suitable and which options of a personalized treatme...
