Chronic abdominal pain and nausea?
I'm taking care of an outpatient patient: 60y/o woman with chronic abdominal pain with prior history of gastric bypass, appendectomy, incisional hernia repair. Her pain had been previously controlled with oxycodone 10mg a couple times a day, but recently less effective due to emesis. GI is following now, has her on Zofran, Reglan, Promethazine but the nausea is still pretty severe. Patient is cachectic and having trouble with PO intake. Utox is appropriate. I have a sense of what I want my career to look like, but in 2017 it's not clear how to get there after completing residency (nearly there...). I'm looking to "do" general surgery (gasp!) -- private vs. local hospital/network -- with a broad swathe of bread and butter as well as more complex cases: Hernias incisional and inguinal, gallbladders, appendectomies, venous access, and a spectrum of thyroid, melanoma, and breast surgery. terrible adhesive allergy, possible iodine allergy connection?
My daughter has terrible reactions (not just a little redness...I mean blistering, weeping) to the tegaderm and adhesives associated with the tune ups and PICC line. She also broke out all over her chest last year in what I think was an allergic reaction to the cleaning agent they used for appendectomy, possibly betadine? Iodine? Does anyone know if there is a link? Please, does anybody have any helpful advice, new products, ANY input on this? Tune up is coming, and we are nervous. Thank you for your help.
