La Isla Incantada
Tomorrow I ' m going to Puerto Rico for a conference, returning Wednesday night. It ' s the International Conference on Communication in Healthcare. I ' m doing a poster and an oral presentation, and moderating three sessions, so I ' ll be busy. I should still have time to post however and if there are any interesting highlights you ' ll get them.My own talk is about experiences of people with substance use disorder with health care. The technical term for my finding is that they are treated like shit. There ' s much more than I can fit in there but I ' ll give you a few quotations from my interviews. These are people who ...

Just say no
I just finished reading Dopesick, by Beth Macy. You don ' t need to take my word that it ' s worth your while, the book was a huge bestseller.  However, it does have a fairly narrow focus. It tells the story of the opioid epidemic largely within the confines of a region of rural Virginia, but this is a disaster of national scope that manifests somewhat differently in different places. Here ' s the really sad news:  source:U.S. Department of Health and Human Services I know it ' s a little small but you can see it full size if you click the link. The short version of the story is that deaths from opioid ...

Codeine cough syrup: a controversial classification
Did you know that promethazine with codeine cough syrup had a high risk of diversion and high risk of addiction? I didn’t. Balancing medical ethics in opioid treatment
What is a legitimate medical reason to prescribe an opioid medication today? For pain or addiction? Who decides which patients are worthy of treatment? This question is not just of academic interest. Out of Control Health Costs or a Broken Society
Flawed Accounting for the US Health Spending Problem By Jeff Goldsmith Source: OECD, Our World in Data Late last year, I saw this chart which made my heart sink. It compared US life expectancy to its health spending since 1970 vs. other countries. As you can see,  the US began peeling off from the rest of the civilized world in the mid-1980’s. Then US life expectancy began falling around 2015, even as health spending continued to rise. We lost two more full years of life expectancy to COVID. By  the end of 2022, the US had given up 26 years-worth of progress in life expectancy gains. Adding four more ...

Addressing dual diagnosis needs in addiction treatment
An excerpt from Slow Dancing with the Devil: A Son’s Substance Use Disorder, A Mother’s Anguish. Vague criteria can lead to misdiagnosis and prison
The definition of addiction, now called substance use disorder, has varied greatly over time. The first edition of the Diagnostic and Statistical Manual, from 1952, didn’t include specific diagnostic criteria for this condition. Over the next sixty years, the DSM went through six revisions, with a slow evolution of the terms “substance abuse” and “dependence.” Read more… Vague criteria can lead to misdiagnosis and prison originally appeared in KevinMD.com.
