Development of an academic-practice partnership model to anchor care coordination and population health
In recent years, the health care delivery system in the United States has undergone a number of revolutionary changes, including a shift from a fee-for-service reimbursement model to a value-based reimbursement model (Putera, 2017; U.S. Department of Health& Human Services, 2016). The Institute for Healthcare Improvement's Triple Aim framework emphasizes the optimization of the U.S. health care system performance by (a) improving the patient experience of care, (b) improving the health of populations, and (c) reducing the per capita cost of health care (Institute for Healthcare Improvement, 2019; Maryland Hospital Association, 2014).
