‘Close’ cardiac monitoring: life-threatening complication of a loop recorder implant

A 76-year-old man with previous coronary bypass surgery, diabetes, and hypertension was admitted following a ‘collapse’. All investigations including cardiac telemetry were unremarkable. A loop recorder (BIOMONITOR III, BIOTRONIK, Germany) was implanted using the standard ‘one-step’ injection technique without apparent complication. He was re-admitted with syncope after 2 weeks; device interrogatio n demonstrated sinus pauses requiring a pacemaker implantation.
