Chronic active T cell –mediated rejection (CA TCMR) is a relatively newly described rejection pathology found in some kidney allografts. CA TCMR is characterized by interstitial fibrosis and tubular atrophy in the presence of features of acute T cell–mediated rejection. To determine the responsiveness of this lesion to immunosuppressive therapy, Kung et al. did a retrospective analysis of 48 cases of isolated CA TCMR. About 92% of the patients were treated with pulse glucocorticoids, antithymocyte globulin, or both.
