Ovarian torsion of large adnexal mass in pregnancy: minimally invasive approach

A 26-year-old at 15 weeks gestational age presented with abdominal pain. Ultrasound showed a 17  × 13 × 22cm clear cyst of uncertain origin that crossed the midline. Pelvic MRI showed ovarian parenchyma compressed at the periphery with an edematous right adnexal vascular pedicle twisted along its axis (Figure 1A, 1B). A CA-125 was not obtained preoperatively due to concern for acute torsion and patient's gravid status. The patient was counseled and proceeded with ovarian detorsion and cystectomy.
