Effect of Tidal Volume on Pulmonary Outcomes After Surgery

To the Editor Dr Karalapillai and colleagues concluded that intraoperative ventilation with low tidal volume compared with conventional tidal volume did not significantly reduce pulmonary complications within the first 7 postoperative days in patients undergoing major surgery. We have some questions about this study. First, the complication rate of 10.8% in the conventional tidal volume group was based on a study that did not mention postoperative pulmonary complications. The estimated 10.8% incidence in the conventional tidal volume group must be referenced to other similar studies on postoperative pulmonary complications.
