Aymptomatic HTN...that actually has damage

So we all know the ACEP policies regarding asymptomatic HTN. But what do you do when that screening comes back positive? Some proteinuria, very mild AKI, etc... Not much in the way of recs on that regard. Especially if the patient doesn't have any established f/u. Admit and subject them to a hefty inpatient bill(especially if uninsured)? Hope they followup in the standard 1-3 days? I've had this dilemma a few times recently since entering the "real world." Obviously not including the people...
