We appreciate Dr. Antinori's interest in our article.1 We agree that our case could be a better example of overlooked and delayed diagnosis of cryptococcosis. Seeing patients who have medical conditions with seemingly apparent diagnosis is one of the “high-risk” scenarios for anchoring, premature closure, and availability bias. In our case, the patient had been already diagnosed with cellulitis by the previous doctor, and there were overt findings of focal inflammation (swelling and redness in her left lower leg); in addition, this completel y resolved with cefazolin within a week.
