Occult Femur Fracture

A 60-year-old woman with poliomyelitis presented with knee pain after falling in the street with her left knee flexed. On examination, the left knee was swollen, with passive motion causing severe pain. The lateral radiograph of the left knee revealed a linear interface between 2 soft tissue densities in the suprapatellar pouch with no evidence of fracture (Figure  1). Arthrocentesis revealed 45 mL of bloody joint effusion with fat globules (Figure 2). Based on the findings of the lateral radiograph and arthrocentesis, a diagnosis of lipohemarthrosis was made.
