Implementing Project Extension for Community Healthcare Outcomes for Geriatric Mental Healthcare in Long-Term Care Facilities

Long-term care (LTC) facilities are being incentivized to shift toward value-based reimbursement, with a focus on achieving the Triple Aim —enhanced patient experience, improved population health, and reduced costs.1,2 High staff turnover is also common in LTC settings, and occupational stress is a major challenge. Thus, achieving the Triple Aim within the LTC context may require addressing a fourth aim of improving the work life of LTC clinicians and staff.3
