Effect of Clinical Pharmacy Services in an Older Adult Emergency Medicine Unit on Unplanned Rehospitalization of Older Adults Admitted for Falls: MUPA-PHARM Study

A significant percentage of the older adult population (25% to 50% of patients older than 85  years) is frail.1 One of the most important symptoms of frailty is falling, which is a risk factor for hospitalization and/or death. Inappropriate prescriptions and polypharmacy are important contributors to the risk for falls.2 The older adult emergency medicine (OAEM) unit in our teaching hospit al treats people older than 75 years with comorbid and complex conditions. The ability of geriatricians and emergency medicine physicians to access clinical pharmacy services is associated with better medication use.
