Intrathoracic Kidney

A 10-month-old girl presented to the emergency department with a 2-day history of fever up to 40 °C and vomiting. Her oxygen saturation was 100% on room air. Findings of a chest examination revealed normal breath sounds. The rest of the physical examination was unremarkable. Her white blood count was 19 200/μL, and a plain radiograph of the chest showed rounded opacity in the left lower lobe (Figure, A and B, arrows). She was treated with oral amoxicillin. Four days later, she returned to the emergency department due to persistence of the fever and a maculopapular rash.
