Transition of Care

In the course of my work as a Palliative Care physician I confront the profound toll of care transitions on family and patient wellbeing daily.Facing the decline and behaviors of a loved one with cognitive impairment is challenging. Home care may not be possible and many families must select a dedicated facility to provide needed care. Specialized residences that commit to provide the best of memory care often appeal to caregivers based on amenities and environment. While the right atmosphere is important, day to day quality of care should drive facility selection.
