Estimating relative efficacy in acute postoperative pain: network meta-analysis is consistent with indirect comparison to placebo alone

Network meta-analysis uses direct comparisons of interventions within randomized controlled trials and indirect comparisons across them. Network meta-analysis uses more data than a series of direct comparisons with placebo, and theoretically should produce more reliable results. We used a Cochrane overview review of acute postoperative pain trials and other systematic reviews to provide data to test this hypothesis. Some 261 trials published between 1966 and 2016 included 39,753 patients examining 52 active drug and dose combinations (27,726 given active drug and 12,027 placebo), in any type of surgery (72% dental). Most trials were small; 42% of patients were in trials with arms

Adams, Akshay LakraABSTRACTHip joint subluxation and dislocations are very common in cerebral palsy (CP) patients and are directly related to the degree of spasticity. Hip dislocation and subluxation leads to hip pain and difficulty in hygiene maintenance by caregiver. Most cases require some sort of surgical intervention to improve the quality of life in these patients. For many years’ pelvic and proximal femoral osteotomies with soft tissue releases was the main stay of treatment for these hips in CP patients. Hip ...

In this study, we investigated the anti-inflammatory protective effect of interleukin-1 receptor antagonist (IL-1ra), an anti-inflammatory cytokine, on systemic lipopolysaccharide (LPS)-induced spinal cord inflammation and oxidative stress, thermal hyperalgesia, and mechanical allodynia in neonatal rats. Intraperitoneal (i.p.) injection of LPS (2 mg/kg) or sterile saline was performed in postnatal day 5 (P5) rat pups, and IL-1ra (100 mg/kg) or saline was administered (i.p.) 5 min after LPS injection. Pain reflex behavior, spinal cord inflammation and oxidative stress were examined 24 h after LPS adminis...

Photo credit Kat-J Dear Carol: My 80-year-old mother had a stroke two years ago and lives in a close-by nursing home. I’m happy to visit her and make sure that her needs are met, but that’s not enough for her. She says it’s my duty to take her to the mall and other places to shop. She’s a large woman and taking her out in this way means getting her in and out of the car as well as wrestling a wheelchair in and out of my car trunk. I have fibromyalgia and body-wide arthritis, but she’s never accepted my health challenges.  I’ve offered to take her for drives since the nursing home st...

Publication date: Available online 24 January 2020Source: Arthroscopy TechniquesAuthor(s): Jeong Woo Kim, Dae Jin NamAbstractThe treatment of massive and irreparable rotator cuff tears remains a challenge for shoulder surgeons. When treating patients with chronic rotator cuff tears, especially those with severe fatty degeneration, severe tendon retraction, or muscle atrophy, the risk of re-tear and persistent severe pain persists. Therefore, surgeons can choose from numerous options. Superior capsular reconstruction (SCR) was introduced as a technique to maintain the stability of the upper shoulder and stabilize the muscle...

Despite the fact that aggressive and the early use of biological and nonbiological disease-modifying antirheumatic drugs have been associated with substantial gains in clinical, radiological, and disability outcomes, a considerable proportion of patients still report significant p...
