A 25-year-old male with end-stage kidney disease from obstructive uropathy and neurogenic bladder presented to the emergency room with generalized tonic-clonic seizures. Two weeks before this presentation, the patient was started on hemodialysis via nontunneled precurved hemodialysis catheter, which was inserted into the right internal jugular vein. A cranial computed tomography obtained for the seizures revealed subcutaneous emphysema of the scalp and air bubbles within the dural venous sinuses (Figure  1a).

Conclusions: Inflammatory pathways associated with pulmonary dysfunction in PLWH suggest multifaceted immune dysregulation involved in different phenotypes of pulmonary dysfunction with a potential specific contribution of the Th17 pathway to airway obstruction in PLWH. Identification of these associations may help in development of treatments that could alter the course of the disease.

ConclusionsAppropriate patient follow-up of clinically significant IFs from lung cancer screening is a well-recognized avenue to improve population health. Education level is a significant independent predictor of appropriate follow-up of IFs, whether as a surrogate for low socioeconomic status or as an indication of health literacy. To address these realities, lung screening shared decision making should adapt to consider health care access and health literacy.
