Confused about what tests to order for altered mental status?

Many readers of The Journal who practiced prior to the advent of the electronic medical record (EMR) will remember charting by hand the chief complaint of “ΔMS” for the child who presented to the emergency department with unexplained delirium, altered mentation, or encephalopathy. ΔMS has now given way to “altered mental status” or “AMS” in the EMR, but has the approach to evaluating the child changed or improved?
