To the Editor — Fusion during entrainment at the cavotricuspid isthmus: When entrainment does not match our expectation

I read with great interest the May 2018 Unknown of the Month by Shen et  al,1 a case of longitudinal dissociation in the cavotricuspid isthmus (CTI) resulting in an unexpected entrainment response. The cycle length (CL) of the CTI-dependent circuit is 281 ms, but the postpacing interval (PPI) from the CTI is 337 ms with a change in coronary sinus (CS) activation during entrainment. During CTI ablation, the tachycardia shifts with CS activation same as during entrainment and CL prolongation to 330 ms.
