Clozapine-Associated Aspiration Pneumonia: Case Series and Review of the Literature. Reply

We read with interest the recent report by Dr. Jonathan Kaplan and colleagues,1 in which they describe 2 patients who developed aspiration pneumonia in the context of clozapine initiation. They rightly underscore the need for clinicians to be alert to the increased risk of pneumonia with the use of antipsychotic medications, especially second generation agents, and review emerging evidence that aspiration pneumonia in particular is a potential complication of treatment with clozapine. In addition to their own cases, they identified 4 prior reports of clozapine-associated pneumonia in which aspiration was strongly implicated.
