I congratulate the authors on their efforts to address the complex issue of whether surgical stabilization of rib fractures (SSRF) in the setting of trauma results in improvements in morbidity and mortality. There were 116 patients from their institution identified as candidates for SSRF, with these patients being compared against a random sampling of 1,000 patients from the National Trauma Data Bank (NTDB) with a similar Injury Severity Score (ISS). The study found that pneumonia and mortality rates were lower for SSRF patient vs those managed nonoperatively, but length of stay, ICU stay, and tracheostomy rates were higher.

We examined the association between MI, medication changes, and post-discharge outpatient visits with 30-day readmission in 40,048 Medicare beneficiaries hospitalized for acute myocardial infarction, heart failure or pneumonia. Beneficiaries with MI were more likely to be readmitted than those without MI (14 vs. 11%). Probability of readmission was 13 and 12% when medications were dropped or added, respectively, versus 11% when no change was made. Probability of readmission also increased with outpatient visits.

Conclusion Serotype data from this study helped in accurate estimation of PCV-13 and PPV-23 protective coverage against serotypes causing IPD in India as 58.7% (95% CI: 53.8, 63.4) and 67.4% (95% CI: 62.7, 71.8) respectively. Penicillin non-susceptibility in meningeal IPD cases is 27.4%. Empirical therapy for meningeal IPD must be cephalosporin in combination with vancomycin since cefotaxime non-susceptibility in meningeal IPD is 9.9%.
