Awardee Evaluation Aids available from NEO

The Outreach Projects Database (OPD) and Activity Reporting (AR) System are platforms for you to share information about your project and project participants. NN/LM funded projects are now required to provide specific information for the following four types of activities: Training Exhibits/Health Fairs Professional Development Technology Improvement If you are applying for NN/LM funding and plan to do any of these activities, you will need to describe in your evaluation plan how and when you intend to collect the required information and any additional evaluation that you are planning to do for your project. For resources to help with evaluation plans, NEO has an Evaluation Resources page with templates and worksheets for evaluation planning, based on the three-booklet series Planning and Evaluating Health Information Outreach Projects.
