62-Year-Old Man With Back Pain and Lower Extremity Weakness

A 62-year-old man presented to the emergency department with progressive lower back pain and bilateral lower extremity weakness and numbness. His medical history was notable for poliomyelitis characterized by residual weakness of the proximal right lower extremity and chronic back pain. He also had a history of multilevel compressive radiculopathy in the lumbar spine. His symptoms had begun 6 weeks previously with both weakness and numbness in the lower extremities, which progressed to difficulty ambulating.
