Editorial: Ex  vivo onco-testis microdissection

When a patient has a solitary testis, a testis mass, and azoospermia, this is the last possible time to obtain sperm from a patient for use in a biologic pregnancy. Options for treatment, in this case, are partial orchiectomy with in  vivo testis microdissection or radical orchiectomy and ex vivo testis microdissection. The decision on what is best is based on shared decision-making with both surgeon and patient based on size of the mass, risk of recurrence with a partial orchiectomy, and the possibility of retaining enough te stis tissue for production of endogenous testosterone and possible spermatogenesis.
