A case of idiopathic retroperitoneal fibrosis (Ormond ’s disease) diagnosed by transduodenal EUS-guided fine-needle aspiration/biopsy

A 71-year-old man presented to our department with back pain and significant weight loss within the previous few weeks. Before referral, ambulatory CT revealed a large retroperitoneal soft tissue mass encircling the abdominal aorta and its surrounding vessels (A). He had mild leukocytosis (11,800/ μL) and a C-reactive protein level of 9.58 mg/dL. The clinical picture was highly suggestive of retroperitoneal fibrosis (RPF); however, for a definite diagnosis, biopsy of the lesion was necessary. We chose transduodenal EUS-FNA/fine-needle biopsy (EUS-FNA/FNB) to obtain tissue because the procedu re is minimally invasive and has already revealed positive results.
