Wide QRS tachycardia with extreme QRS right-axis deviation

A 64-year-old man presented with epigastric discomfort and palpitations after consuming a festive meal. He reported taking no medications. Examination results were normal (with blood pressure of 120/70 mm Hg) except for tachycardia (Figure  1). What is the most likely diagnosis of the tachycardia?
