A Wide-QRS Tachycardia with an Extreme QRS Right Axis Deviation

A 64-year-old man presented with epigastric discomfort and palpitations after a festive meal. He reported taking no medications. His examination was normal (with a blood pressure of 120/70mmHg) except for tachycardia (Figure 1).
