Community considerations for aggressive intensive care therapy for infants < 24+0 weeks of gestation

Considerable recent discussion has focused on recommendations to lower the gestational age (GA) at which active intervention is instituted to 22 weeks.1-5 For the neonatal community to sensibly adopt such proposed changes, a concerted effort is needed to engage more widely, not only within its own profession, but also with representatives of the affected families and with professionals providing ongoing, long-term services to the families we serve. As an international group of neonatologists, ethicists, and parent advocates we aim to encourage that individualized decision-making with families takes precedence over any mandatory policies.
