Health care transition is associated with poor health outcomes if not planned. Moving to college is a significant life event for many emerging adults, especially those with special health care needs. For students living with type 1 diabetes (T1D), the transition to both adult care and college is often characterized by stress and difficulty self-managing the disease. The Health Care Transition Research Consortium model seeks to provide a theoretical framework for the transition experiences of adolescents; however, it does not consider the ancillary health services available at or near college campuses.
