We appreciate the authors ’ interest in our recently published “ACR Appropriateness Criteria® Headache: 2022 Update.” The authors raise an important consideration with respect to the use of head CT angiography (CTA) for patients presenting with sudden-onset severe headache that reaches maximal intensity within 1 hour (Variant 1). Although we stand by our “may be appropriate” recommendation, we agree that a more nuanced discussion as to the benefits and drawbacks of CTA versus lumbar puncture in patients presenting after 6 hours is important.
