Policy and Process Considerations for the Updated Joint Commission Fluoroscopy Sentinel Event Definition

In July 2021 the Joint Commission (JC) announced an update to its “fluoroscopy sentinel event” (SE) definition, which became effective January 1, 2022 [1]. The previous definition of an SE as a total (cumulative) dose of 1,500 rads (15 Gy) to a single field was, in theory, straightforward, empirical, and derivable. However, complying with the former definition required estimating the magnitude and distribution of radiation dose across the skin surface using numerous technical parameters [2], involving substantial uncertainty.
