We thank Zhang et al1 for their interest in our recently published study,2 which concluded that sparing of oblique fibers after peroral endoscopic myotomy (POEM) does not reduce the incidence of gastroesophageal reflux (GER). Instead of evaluating modified techniques to prevent GER, the authors suggested that futur e research should explore the pathogenesis, risk factors, and treatment of post-POEM refractory GER.
