Retroperitoneal ectopic pregnancy, a concealed and threatened ectopic pregnancy

A 26 year old G3P2 presented complaining of back pain for 10 days and amenorrhea for 64 days. Her blood Human Chorionic Gonadotropin (hCG) level was 35759U/L. Transvaginal ultrasound showed neither intrauterine gestational sac nor adnexal mass. An abdominal CT scan revealed a gestational sac located between the two great vessels and immediately adjacent to the vena cave [Fig A].
