Management of Portal Biliopathy

AbstractPurpose of ReviewPortal biliopathy or portal cavernoma cholangiopathy is defined by the presence of cholangiographic abnormalities in a patient with portal cavernoma. In this review, we will examine the evaluation and management of symptomatic portal biliopathy in light of the significant breakthroughs that have been made in this field.Recent FindingsThe imaging modality of choice for evaluating portal biliopathy is magnetic resonance cholangiography with portography. Endosonography and cholangioscopy may provide complementary information regarding the etiology of biliary obstruction and should be considered especially when endotherapy is contemplated.SummaryAsymptomatic patients with portal biliopathy do not require an active intervention. In symptomatic patients with cholangitis and biliary calculi, endotherapy is often the initial treatment. However, portosystemic shunt surgery +/ − bilioenteric anastomosis (if required) is the definitive treatment in those with shuntable veins. Future studies are required with regard to the management in cases with extensive thrombosis of portal venous system.
