The course structure varies hugely between medical schools, so it's impossible to give an overall answer. Some will split it by topics (anatomy, pathology, etc), others by systems (cardiac, respiratory...) and the methods used and amount of clinical exposure, especially early on, will vary hugely. I strongly suggest you have a read of some universities' websites to find out about the courses, and many of them will have videos/blogs from students as well, to hear their versions.
