The long-term impact of perioperative red blood transfusion on patients undergoing cardiac surgery.
CONCLUSIONS: Patients with perioperative PRBC transfusions have increased operative and long-term mortality and hospital readmissions. Total units of PRBC transfused was directly associated with mortality and readmissions. PMID: 33171175 [PubMed - as supplied by publisher]

The Impact of Race on Outcomes of Revascularization for Multivessel Coronary Artery Disease.
CONCLUSIONS: Black patients with MVCAD have a higher comorbidity burden and undergo PCI at higher rates. After adjusting for baseline differences, black patients still had higher rates of PCI utilization and long-term stroke. It is possible that a significant portion of racial disparities in MVCAD are driven by differences in baseline risk, however, there is evidence of possible racial bias with regards to revascularization strategies. Outcome of CentriMag ™ extracorporeal mechanical circulatory support use in critical cardiogenic shock (INTERMACS 1) patients
ConclusionOur results demonstrate an excellent outcome with the use of the CentriMag ™ device in this seriously ill population. Despite requiring multiple procedures, over 58% of patients were discharged from hospital with 5-year survival of 46%.

ConclusionsThis is the first study reporting the long ‐term impact of peripheral cannulation for redo cardiac surgery after excluding patients with operative mortality. These data suggest that central cannulation may to be the preferred approach to redo cardiac surgery whenever safe and possible.

Mitral valve prosthesis choice in patients < 70 years: A systematic review and meta ‐analysis of 20 219 patients
ConclusionMechanical MVR in patients aged less than 70 years is associated with a lower risk of operative mortality as well as a 20% lower risk of long ‐term death and 65% lower risk of mitral valve reoperation but 60% greater risk of bleeding compared with bioMVR in matched or adjusted data.

Effects and outcomes of cardiac surgery in patients with cardiometabolic syndrome
ConclusionsPatients with CMS were more likely to present with increased comorbidities. Patients with CMS undergoing CABG were at risk for worse short ‐term secondary postoperative outcomes and reduced long‐term survival. The data supports the need for further investigation for risk reduction surrounding operative revascularization.

ConclusionsParasternal minimally ‐invasive aortic valve replacement is a feasible technique associated with a slow learning curve but the potential to achieve improved outcomes. Considering the transcatheter alternatives, the relative risk reduction may be worth investigating in future trials.

Predictors and outcomes of stroke after transcatheter aortic valve replacement
ConclusionsAlternative access approach and a history of transient ischemic attack emerged as independent predictors of postoperative stroke. Patients with stroke suffered more complications and had worse survival, underscoring the importance of characterizing the stroke risk in these patients.

Bilateral versus single internal thoracic artery grafting in hemodialysis patients
ConclusionsBITA grafting was able to achieve revascularization with minimal manipulation of the diseased ascending aorta without increasing the complication rate. The long-term survival benefit of BITA grafting, however, was unclear in dialysis patients, especially because such patients have a relatively short life expectancy.

TAVR After Prior Mitral Valve Surgery: Results from the Transcatheter Valve Therapy Registry
ConclusionsPatients with prior SMVR undergoing TAVR had similar 30-day outcomes, slightly higher 1-year mortality, and no increase in early PVL compared to those without previous SMVR. Prior SMVR should not preclude TAVR in appropriately selected patients.
