A Novel Mobilization Criteria Checklist 12 to 24 Hours After Intravenous Thrombolysis in Acute Ischemic Stroke
This study describes the development and utilization of a novel mobilization criteria checklist that can guide clinical decision making to safely initiate very early mobilization for patients with acute ischemic stroke within 12 to 24 hours after intravenous (IV) thrombolysis. Methods: The evidence was examined, and risk factors for hemorrhagic transformation and considerations for initiating or terminating mobility within the first 12 to 24 hours were identified. The information was formatted into a mobilization criteria checklist for use in a case study 12 to 24 hours after tissue plasminogen activator (tPA), an I...

Two Balance Measures as Poststroke Predictors of Ambulation Status at Discharge From Inpatient Rehabilitation
This study aimed to (1) determine whether the Postural Assessment Scale for Stroke (PASS) and the Berg Balance Scale (BBS) can predict an individual's IRF discharge poststroke gait category and (2) establish cut-off scores for these assessments to better forecast gait category upon discharge. Methods: A total of 180 individuals with an admitting stroke diagnosis were assessed with both balance measures and the 10-m walking test at admission and discharge from inpatient rehabilitation. Participants were stratified by gait speed into 4 groups (nonambulators: 0 m/s; household ambulators: 0.8 m/s). Results: Both ...

Current Clinical Practices of Saudi Physiotherapists in Stroke Rehabilitation
This article uses an existing American survey to compare Saudi physiotherapists' current practice and preferred interventions in stroke rehabilitation with their counterparts in other countries. Method: An electronic survey was sent between September and October 2020 to physiotherapists (n = 287) treating persons with stroke in Saudi Arabia. The questionnaire consisted of items related to stroke rehabilitation, including treatment approach, the goal of interventions, management of tone, facilitation of movement, function, and improved motor control. Chi-square tests (χ2) were used to test any associations between p...

Body Weight–Supported Treadmill Training in the Acute Care Setting
Conclusions: BWSTT is a safe and feasible intervention to assist with higher-level physical activity in the acute stroke population during the early recovery stages.

Implementing a Rehabilitation Protocol for Spatial Neglect Assessment and Treatment in an Acute Care Hospital
Conclusions: Implementation of a standard rehabilitation assessment and treatment protocol for spatial neglect in acute care is feasible. The findings related to fidelity are promising; however, further research is recommended.

Mobilization for Persons With Acute Stroke: A Survey of Current Physical Therapy Clinical Practice
Conclusions: The frequency of mobilization reported in this survey is less than proposed by the results of the most recent “A Very Early Rehabilitation Trial” (AVERT) after stroke, phase 3 trial. This study found a positive correlation between increased session frequency and recovery of walking at 3 months, survival, and a modified Rankin Scale (mRS) score of 2 or less. Respondents believe additional mobilization would be beneficial, but report barriers to implementation that include increased PT caseloads and lack of resources. Additional research is required to determine the influence of mobilization timing and fre...

Providing Culturally Competent Acute Care Physical Therapy for a Complex Medical Patient of Amish Heritage
Discussion: Physical therapists may be unfamiliar with patients of Amish heritage due to the infrequency of PT services used by these individuals. This case report demonstrates a physical therapist's sensitivity to and acceptance of the Amish cultural beliefs, which ensured appropriate clinical decisions and positive patient- and family-centered outcomes. Engaging the patient's family in dialogue about realistic discharge options early in the acute care stay allowed the family time to understand the complexity of the patient's diagnoses and challenges of the health care environment as well as align decisions with their cultural beliefs.

Use of the Postural Assessment Scale for Stroke Patients in Determining Acute Care Discharge Recommendations
Conclusions: One hundred fifty-three subjects, with a mean age of 64.5 ± 15.5 years, were included in this cohort. The mean PASS score was 19.7 ± 11.1, the mean mRS score was 3.7 ± 2.2, and the mean FIM scores was 2.7 ± 2.4 for transfers and 3.7 ± 1.6 for ambulation. Of the persons discharged home (N = 40), mean PASS scores were 31.8 for those recommended for home physical therapy (N = 14), 32.6 for an outpatient therapy recommendation (N = 12), and 34.9 for those requiring no therapy at discharge (N = 14). The mean PASS scores for persons with an acute rehabilitation recommendation (N = 100) was higher than those rec...

Outcome Measures for Persons With Acute Stroke: A Survey of Physical Therapists Practicing in Acute Care and Acute Rehabilitation Settings
Conclusions: Comfort, evidence, resources, and time influence OM use by physical therapists for persons with stroke. Use of StrokEDGE highly recommended OMs is associated with practice setting, but not with years of practice, APTA membership, or specialist certification.
