An interrupted time series study of electronic health record clinical decision support for providers caring for patients with atrial fibrillation at increased stroke risk
CONCLUSION: There was a significant trend of increasing anticoagulation rates during the pre-BPA period, with no significant change in trend during the post-BPA period relative to the pre-BPA period.PMID:37696764 | DOI:10.1093/ajhp/zxad188

A practice game changer: Impact of tenecteplase for acute ischemic stroke in a multicenter quality improvement project
CONCLUSION: Institutions can safely transition from alteplase to tenecteplase as a thrombolytic of choice for AIS.PMID:35037028 | DOI:10.1093/ajhp/zxab482

Tenecteplase use in the management of acute ischemic stroke: Literature review and clinical considerations
CONCLUSION: Current clinical evidence shows that tenecteplase is not inferior to alteplase for the treatment of ischemic stroke and suggests that tenecteplase may have a superior safety profile. Furthermore, tenecteplase also has practical advantages in terms of its administration. This can potentially lead to a decrease in medication errors and improvement in door to thrombolytic time.PMID:35020806 | DOI:10.1093/ajhp/zxac010

COVID-19 preparedness: Clinical pharmacy services remote staffing in a quaternary, level I trauma and comprehensive stroke center
CONCLUSION: The proactive measures described can help ensure that pharmacy caregivers have appropriate remote access and are capable of confidently using the resources. These steps allow for optimal care of hospitalized patients and promote social distancing, which may have the added benefit of decreasing the spread of SARS-CoV-2 among patients and caregivers.PMID:34286818 | PMC:PMC7239242 | DOI:10.1093/ajhp/zxaa132

Development of pulmonary embolism in a nonhospitalized patient with COVID-19 who did not receive venous thromboembolism prophylaxis.
CONCLUSION: This case suggests that nonhospitalized patients with COVID-19 may be at higher risk for VTE than patients with other medical illnesses and warrants further research into the risk of VTE in outpatients with COVID-19. PMID: 32780839 [PubMed - as supplied by publisher]

Direct-acting oral anticoagulant use at extremes of body weight: Literature review and recommendations.
CONCLUSION: While the published evidence on use of DOACs in patients at extremes of body weight is sparse, apixaban and rivaroxaban appear to have the most favorable safety and efficacy profiles. Edoxaban and dabigatran should be avoided. PMID: 32426845 [PubMed - in process]

A systematic overview of systematic reviews evaluating medication adherence interventions.
Conclusions regarding intervention effectiveness were extracted. Grades of Recommendation, Assessment, Development and Evaluation (GRADE) methodology was applied to assess evidence quality. RESULTS: Of 390 SRs, 25 met the inclusion criteria and assessed adherence as a primary outcome. Intervention types most consistently found to be effective were dose simplification, patient education, electronic reminders to patients, and reduced patient cost sharing or incentives. Of 50 conclusions drawn by the SRs, the underlying evidence was low or very low quality for 45 SRs. CONCLUSION: Despite an abundance of primary stud...

Comparison of real-world outcomes in patients with nonvalvular atrial fibrillation treated with direct oral anticoagulant agents or warfarin.
Conclusion: The results of the study indicated that patients on DOACs had lower rates of ischemic stroke, hemorrhagic stroke, SE, and composite outcome of stroke or SE compared with patients on warfarin. No significant differences in bleeding rates between the DOAC and warfarin groups were observed, while total cost of care was lower in the DOAC group. Hemorrhagic conversion after alteplase administration in a patient with vasculitis and acute ischemic stroke.
Conclusion: More data regarding alteplase treatment in patients with GPA are needed to further establish the safety of this therapy. PMID: 30689698 [PubMed - as supplied by publisher]

Transdermal estradiol for the management of refractory uremic bleeding.
CONCLUSION: Patients who are refractory to routine interventions for uremic bleeding may benefit from transdermal estrogen despite the limited data. Extended therapy with low-dose transdermal estrogen (≤50 μg daily) may provide a hemostatic benefit that outweighs thrombotic risk. PMID: 29691259 [PubMed - in process]
