Outcomes of Root Enlargement Versus Root Replacement for Aortic Stenosis
CONCLUSIONS: ARE and stentless xenograft ARR for aortic stenosis were associated with comparable postoperative complications, aortic valve reinterventions, freedom from readmission, and 5-year survival.PMID:36584836 | DOI:10.1016/j.athoracsur.2022.12.027

Cardiac Operations after Transcatheter Aortic Valve Replacement
CONCLUSIONS: Cardiac operations after TAVR are increasing and interval between TAVR and operation decreasing. Most cardiac operations are complex, high-risk reoperations and isolated AVR rare. These findings should be considered when TAVR is selected for low-intermediate risk patients, particularly with multiple cardiac pathologies not addressed by TAVR.PMID:34800488 | DOI:10.1016/j.athoracsur.2021.10.022

TAVR After Prior Mitral Valve Surgery: Results from the Transcatheter Valve Therapy Registry.
CONCLUSIONS: Patients with prior SMVR undergoing TAVR had similar 30-day outcomes, slightly higher 1-year mortality, and no increase in early PVL compared to those without previous SMVR. Prior SMVR should not preclude TAVR in appropriately selected patients. PMID: 31655043 [PubMed - as supplied by publisher]

Aortic Valve Replacement in Bioprosthetic Failure:Insights from The Society of Thoracic Surgeons National Database.
CONCLUSIONS: The number of patients undergoing SAVR for degenerated bioprosthesis is decreasing in U.S, particularly among older and high-risk patients. These trends may reflect the adoption of valve-in-valve transcatheter aortic valve replacement for degenerated bioprosthesis after its FDA approval in 2015. Outcomes of Isolated Tricuspid Valve Surgery Have Improved in the Modern Era.
CONCLUSIONS: In the current era with careful patient selection and periprocedural management, isolated TV surgery can be performed with lower morbidity and mortality than has traditionally been reported with good long term survival. These outcomes can also serve as a benchmark for catheter-based tricuspid valve intervention outcomes. PMID: 30951698 [PubMed - as supplied by publisher]

Does Mitral Valve Calcium In Patients Undergoing Mitral Valve Replacement Portend Worse Survival?
CONCLUSIONS: Conservative approach to treat MAC achieves satisfactory results. Patients with MAC have significant comorbidities contributing to a worse survival though MAC in itself is not a risk factor for mortality. PMID: 30391339 [PubMed - as supplied by publisher]

Clinical Outcomes of Mitral Valve Reoperations in the United States: An Analysis of the Society of Thoracic Surgeons National Database.
CONCLUSIONS: Despite a high-risk patient profile, surgical outcomes of reoperative MVS were acceptable, particularly in elective cases with prior mitral valve surgery and without endocarditis. PMID: 30365952 [PubMed - as supplied by publisher]

Preoperative Venoarterial Extracorporeal Membrane Oxygenation Slashes Risk Score in Advanced Structural Heart Disease.
CONCLUSIONS: ECMO can be used as a bridge to heart valve or septal defect surgery in severely decompensated patients. Through recovery of end-organ function, ECMO may allow surgical correction of structural heart disease in patients considered inoperable or convert a salvage situation to an elective operation. Outcomes of Planned 2-Stage Hybrid Aortic Repair with Dacron Replaced Proximal Landing Zone.
CONCLUSIONS: Planned 2-stage HAR with open 1st stage PAR followed by 2nd stage TEVAR with Dacron replaced PLZ yields excellent short and long-term results, including low rates of re-intervention likely due to long-segment PLZ within Dacron-replaced aorta. The technique should be considered in patients with even mild (>4.0 cm) ascending aortic dilation in whom HAR is otherwise the preferred treatment option. Hemiarch Reconstruction Vs Clamped Aortic Anastomosis for Concomitant Ascending Aortic Aneurysm.
CONCLUSIONS: Hemiarch replacement using DHCA with RCP does not increase the risk of operative complications compared to a normothermic, clamped-distal aortic anastomosis, and therefore its use should not be limited when planning complex multi-procedural reconstructions during elective ascending thoracic aortic replacement with concomitant cardiac surgery. Outcomes of Dialysis-Dependent Patients After Cardiac Operations in a Single-Center Experience of 483 Patients.
CONCLUSIONS: Patients with DD CRF undergoing cardiac operations have high perioperative and medium-term mortality, particularly in the presence of combined aortic and mitral valve pathologic conditions, active endocarditis, and poor left ventricular function. Prolonged Cross-Clamping During Aortic Valve Replacement Is an Independent Predictor of Postoperative Morbidity and Mortality: Analysis of the Japan Cardiovascular Surgery Database.
CONCLUSIONS: Prolonged ACCD offers an independent predictor of postoperative morbidity and mortality after isolated AVR for AS despite recent technologic advances and surgical refinements. PMID: 27624296 [PubMed - as supplied by publisher]

Aortic Valve Replacement in the Moderately Elevated Risk Patient: A Population-Based Analysis of Outcomes.
CONCLUSIONS: This population-based contemporary assessment suggests moderate-risk patients undergoing AVR experience favorable outcomes. Although increasing PROM is important in preoperative evaluation of risk, preexisting pulmonary hypertension and indication for operation are among other factors that should be considered as TAVR expands into this group of patients.
