Age as a prognostic factor in relation to surgical evacuation of spontaneous supratentorial intracerebral haemorrhage
Conclusions and clinical implicationsNeurosurgical treatment of the sICH patients is indicated only if age and potentially improved morbidity is carefully evaluated considering the STICH and this study; otherwise, we will just increase the health care burden with a number of extremely care-dependent patients.

sP- and sE-selectin in stroke patients with metabolic disorders
ConclusionSoluble E- and P-selectin, blood markers of vascular injury and platelet activation, could be useful in the assessment of atherothrombotic properties of hyperglycemia and hyperlipidemia in stroke patients.

Cerebral thrombolysis in patients with ischemic stroke and heart failure
ConclusionThe presence of systolic HF, defined according to an old and a new definition, does not determine safety and efficacy of cerebral iv-thrombolysis in patients with IS.

MMP-9 and/or TIMP as predictors of ischaemic stroke in patients with symptomatic and asymptomatic atherosclerotic stenosis of carotid artery treated by stenting or endarterectomy – A review
Publication date: Available online 30 May 2018Source: Neurologia i Neurochirurgia PolskaAuthor(s): Justyna Zielinska-Turek, Małgorzata Dorobek, Grzegorz Turek, Maria Barcikowska-KotowiczAbstractWe still lack an optimal tool to predict ischaemic stroke in patients with symptomatic and asymptomatic carotid stenosis (CS). It has already been shown that patients at increased risk of ischaemic stroke can be identified based on the elevated plasma levels of metalloproteinases (MMPs) and reduced activity tissue inhibitor of metalloproteinase (TIMP). There are few studies presenting the role of MMP-9 and TIMP in ischaemic stroke ...

Future challenges of stroke treatment
Publication date: May–June 2018Source: Neurologia i Neurochirurgia Polska, Volume 52, Issue 3Author(s): Simona Lattanzi, Mauro SilvestriniAbstractPosterior circulation stroke accounts for approximately 20% of all ischaemic strokes. Acute basilar artery occlusion (BAO) is one of the most severe conditions, it is associated with death or major disability in more than three quarters of the cases, and its optimal management remains unestablished. Currently, the treatment is based primarily upon consensus, the clinical practice varies widely, and the actual benefit of mechanical thrombectomy has to be fully estimated. Althoug...

BOLD fMRI signal in stroke patients and its importance for prognosis in the subacute disease period – Preliminary report
ConclusionThere are differences observed in the activation areas of the cerebral cortex both in the stroke and non-stroke hemispheres. More than half of the patients with hemispheric stroke but all with good outcome showed cerebellar activation. There is probable positive correlation between the BOLD-signal size, young age, activation of supplementary motor area in stroke hemisphere and good functional status of patients in the subacute period of stroke.

Looking for novel, brain-derived, peripheral biomarkers of neurological disorders
Publication date: May–June 2018Source: Neurologia i Neurochirurgia Polska, Volume 52, Issue 3Author(s): Natalia Chmielewska, Janusz Szyndler, Karolina Makowska, Dawid Wojtyna, Piotr Maciejak, Adam PłaźnikAbstractThe role of blood brain barrier (BBB) is to preserve a precisely regulated environment for proper neuronal signaling. In many of the central nervous system (CNS) pathologies, the function of BBB is altered. Thus, there is a necessity to evaluate a fast, noninvasive and reliable method for monitoring of BBB condition. It seems that revealing the peripheral diagnostic biomarker whose release pattern (concentratio...

Malignant middle cerebral artery (MCA) infarction in people over 85 years old – Diagnosis, management and risk factors
ConclusionThe key risk factor in this group is the atrial fibrillation. The elderly patients require an intensive monitoring of the health condition by reference to brain stroke risk factors, especially atrial fibrillation.

Tako-tsubo cardiomyopathy with transient global amnesia and cerebellar embolic stroke triggered by existential fear
ConclusionsTGA is not a disease for the neurologists exclusively but also cardiac comorbidities have to be considered and cardiologists should be involved in the acute care of these patients in order not to overlook life-threatening diseases.

Cerebral vasomotor reactivity predicts the development of acute stroke in patients with internal carotid artery stenosis
ConclusionDecreased VMR value is an important prognostic factor for the occurrence of aortic atherosclerotic stroke, and can be used as a reference for preoperative hemodynamic evaluation in patients with internal carotid artery stenosis.

Mechanical thrombectomy: Determining the proportion of eligible acute ischemic stroke patients in the cohort of single academic stroke center
ConclusionAmong AIS patients, in whom onset-to-arrival time allowed to initiate the endovascular procedure within 6 h of symptom duration, 21% were eligible for MT treatment according to more and 33% to less restrictive criteria. It accounts for about 6% and 9% of all AIS cases, respectively.

Alpha-2-antiplasmin Arg407Lys polymorphism and cryptogenic ischemic cerebrovascular events: Association with neurological deficit
ConclusionThe presence of 407Lys allele is associated with better prognosis in cryptogenic cerebrovascular events. Our findings suggest that the α2AP Arg407Lys polymorphism could be involved in the pathogenesis of cerebral ischemia and its outcomes.

Symptomatic medulla compression by vertebral artery
ConclusionVAMCS should be considered as the cause of neurological deficits when other pathological entities are ruled out. In symptomatic conflict of the VA with the medulla, microvascular decompression using a Gore-Tex implant can be an effective method of treatment. Nevertheless, a statistical analysis on all reported cases showed favorable results using the VA repositioning technique when compared with MVD (success rate 91% vs. 58%, p < 0.05).
