Transapical and Transaortic Transcatheter Aortic Valve Replacement in the United States.
CONCLUSIONS: Patients undergoing TAo TAVR are older, more likely female, and have significantly higher STS predicted risk of mortality scores than patients operated on by TA access. There were no risk-adjusted differences between TA and TAo access in mortality, stroke, or readmission rates as long as 1 year after TAVR. PMID: 26233276 [PubMed - as supplied by publisher]

Outcome of redo surgical aortic valve replacement in patients ≥ 80 years: results from the multicenter RECORD initiative.
CONCLUSIONS: Octogenarians undergoing S-AVR after prior cardiac surgery have similar immediate postoperative outcome to younger patients and their 5-year outcome is excellent. Variation in Warfarin Thromboprophylaxis After Mitral Valve Repair: Does Equipoise Exist and Is a Randomized Trial Warranted?
CONCLUSIONS: At present, half of patients are prescribed warfarin after isolated mitral valve repair in North American cardiac surgical practice which may impact the length of hospital stay. Although patient-level predictors of warfarin prescription exist, center- and surgeon-level variations are prominent. Hemiarch and Total Arch Surgery in Patients With Previous Repair of Acute Type I Aortic Dissection.
CONCLUSIONS: In patients with previous acute type I aortic dissection repair, hemiarch and total arch operations have respectable morbidity and survival rates. Congestive heart failure predicts operative death, long-term mortality, and our adverse event endpoint. Preoperative Venoarterial Extracorporeal Membrane Oxygenation Slashes Risk Score in Advanced Structural Heart Disease.
CONCLUSIONS: ECMO can be used as a bridge to heart valve or septal defect surgery in severely decompensated patients. Through recovery of end-organ function, ECMO may allow surgical correction of structural heart disease in patients considered inoperable or convert a salvage situation to an elective operation. Aortic Annular Enlargement in the Elderly: Short and Long-Term Outcomes in the United States.
CONCLUSIONS: These data suggest annular enlargement during SAVR is associated with increased short-term risk in a Medicare population. Survival curves crossed after three years, which may portend a benefit in select patients. However, annular enlargement is still only done in the minority of patients who are at risk for PPM. CONCLUSIONS: In well-selected patients, early discharge after minimally invasive valve surgery was associated with lower costs but comparable postoperative outcomes.
