ConclusionsEndovascular repair of the MCA aneurysm inclosing those with wide neck and complex are not only safe and feasible, but associated with high long-term functional outcome. P-001 Permanent Implantation of the Solitaire Device as a Bailout Technique for Large Vessel Intracranial Occlusions
ConclusionWith the recent over-whelming evidence demonstrating the effectiveness endovascular therapy in acute ischemic stroke, the expansion of stroke therapy requires that a variety of techniques be available to the operators. Permanent implantation of the Solitaire stent achieves adequate distal flow and good clinical outcomes in the setting of AIS. Delayed infarction following aneurysmal subarachnoid hemorrhage: Can the role of severe angiographic vasospasm really be dismissed?
Conclusions Severe angiographic vasospasm may be linked to infarction in patients who present late. These infarcts are mostly large despite maximal treatment. We question the notion that proximal vasospasm has a minor role in delayed ischemia.

Stagnation of treatment times over a decade: results of a pooled analysis from the MERCI registry, MERCI, TREVO, and TREVO 2 trials
Conclusions Despite a reduction in procedure times, there has not been a corresponding improvement in overall last known normal to puncture times over a 10-year period. The current study shows that there are many opportunities to create more efficient endovascular stroke systems of care in trials.

Collateral flow as causative of good outcomes in endovascular stroke therapy
Conclusions Improved collateral flow in patients with AIS undergoing endovascular therapy was associated with improved radiographic and clinical outcomes. Independent of age, vessel occlusion and time, in patients with comparable ischemic burdens, changes in collateral grade alone led to significant differences in initial stroke severity as well as ultimate clinical outcome.

Correlation between cerebral blood volume values and outcomes in endovascular therapy for acute ischemic stroke
Conclusions We found that cortical CBV loss is predictive of poor clinical outcomes, whereas basal ganglia CBV loss is predictive of hemorrhagic transformation but without translation into poor clinical outcomes. Our study findings support published results of baseline preintervention CBV as a predictor of outcomes in patients undergoing intra-arterial stroke therapies.

Balloon remodeling of complex anterior communicating artery aneurysms: technical considerations and complications
Conclusions Balloon remodeling should be considered for broad-based complex ACoA aneurysms. This technique provides a high rate of aneurysm occlusion with an acceptable complication profile, and avoids the need for dual antiplatelet therapy. The balloon trajectory will depend on aneurysm morphology and bilateral access may be useful in selected cases.

Methamphetamine use is an independent predictor of poor outcome after aneurysmal subarachnoid hemorrhage
Conclusions Methamphetamine users have significantly worse outcomes at 1 and 3 years following aSAH. Further analysis is necessary to understand the pathological response associated with methamphetamine use in this setting.

Modifying flow in the ACA-ACoA complex: endovascular treatment option for wide-neck internal carotid artery bifurcation aneurysms
Conclusions We found that the proposed technique of flow modification can allow for hemodynamic conversion of ICAb to ‘side-wall’ aneurysm. In patients with good collateral flow through the anterior communicating complex, this treatment paradigm is safe and effective.
