E-074 Case report: reconstructive endovascular treatment of symptomatic long segment dissection with pseudo-aneurysm of dominant left vertebral artery
We report a case of 27 year old Caucasian female with no known medical history who presented to our hospital with several days’ history of posterior headache and neck pain. CT head was unremarkable. CTA head and neck showed short segment of severe stenosis secondary to dissection at the V2-V3 junction of dominant left vertebral artery with associated 6 mm pseudo aneurysm. Right vertebral artery was hypoplastic and it ended in PICA. The dissection was presumed to be spontaneous as patient denied any trauma or strenuous neck manipulation or family history of connective tissue disorders. She was started on antiplatelet ...

E-091 Intracranial atherosclerotic disease leads to poor mechanical thrombectomy outcomes in cocaine associated acute ischemic stroke with large vessel occlusions
Conclusion: In this case series, intracranial atherosclerosis secondary to cocaine use and vasospasm contributed to unsuccessful recanalization and reocclusion in patients undergoing MT with poor outcomes. Further studies are needed to ascertain strategies for improved outcomes in patients with LVO related to cocaine use.Disclosures M. Memon: None. M. Kushnirsky: None. S. Koch: None. D. Yavagal: None.

Prophylactic administration of tirofiban for preventing thromboembolic events in flow diversion treatment of intracranial aneurysms
Conclusions The current study suggested that prophylactic administration of tirofiban combined with conventional oral DAT seems safe and efficient for preventing TEs during FD treatment of unruptured intracranial aneurysms. Balloon angioplasty and prolonged procedure are associated with a high risk of TEs.

E-004 Focal intracranial vasculopathy as a manifestation of COVID-19-associated acute ischemic stroke
ConclusionTo our knowledge, our case is the first to illustrate the potential for COVID-19 infection to present as a focal intracranial vasculopathy in an otherwise healthy young-patient, resulting in acute ischemic stroke without an underlying hypercoagulable state. Rescue intracranial stenting was necessary to maintain vessel patency and restore intracranial flow.Disclosures A. Mahapatra: None. A. Witek: None. G. Toth: None.
