Reocclusion after successful endovascular treatment in acute ischemic stroke: systematic review and meta-analysis
Conclusion In cohorts of patients with AIS due to LVO, 5% of patients experienced reocclusion within 7 days after successful EVT. Repeated EVT can be a safe and effective treatment for selected patients with reocclusion.

Post-reperfusion hyperperfusion after endovascular stroke treatment: a prospective comparative study of TCD versus MRI
Conclusions TCD is a reliable bedside tool to identify post-reperfusion hyperperfusion, correlates well with perfusion MRI, and indicates risk of reperfusion injury after MT.

Thrombus radiomics in patients with anterior circulation acute ischemic stroke undergoing endovascular treatment
Conclusions TR describing thrombus volume and texture were associated with more attempts to successful reperfusion. Compared with models using CV and MTM, TR had no added value for predicting procedural and functional outcome.

O-034 Women with large vessel occlusion acute ischemic stroke are less likely to be routed to comprehensive stroke centers
ConclusionsLVO patients who presented to PSCs were less likely to receive EVT than patients directly presenting to a CSC. Anterior circulation LVOs were more likely to be transferred for EVT evaluation. Despite a higher NIHSS, and similar other baseline characteristics, female patients were less likely to be routed to a CSC.Disclosures M. Tariq: None. S. Salazar-Marioni: None. S. Khose: None. L. E-021 Pathophysiology, stroke types, collateral formation and aneurysms in moyamoya disease/moya moya syndrome
ConclusionsMMD is presented most commonly with ischemic stroke in the pediatric age group and with hemorrhagic stroke in the adult age group. MMD-associated intracranial aneurysms frequently occur in patients presenting with hemorrhagic unilateral MMD and are associated with an extremely high rate of rupture. Long-standing hemodynamic stress might contribute to the formation of an aneurysm.Disclosures S. Ahmad: None.

E-037 What Stroke Patients Post Online About: An Instagram and Twitter Analysis
ConclusionSocial media has provided a platform for analyzing unilateral feedback from patients, concurrently as they share their personal experiences and connect with others. Raising awareness and describing their quality of life were the most common themes online. Such platforms help assess patients’ satisfaction and guide physicians for better counseling aiming to improve the quality of provided care.Disclosures A. Gajjar: None. A. Jain: None. A. O-031 Implementation of an Institutional-Wide Acute Stroke Algorithm: Improving Stroke Quality Metrics
Conclusion A stroke protocol was successfully implemented at our institution with promising results. Several QI parameters significantly decreased. We describe our algorithm for the benefit of other developing stroke centers in their mission to improve care of the acute stroke patient. E-030 Incidence and risk factors for new seizure after ischemic stroke in a single institution series
DiscussionThe literature on new onset seizures after stroke describes a range of incidences and risk factors, as there is variation in the studied populations. Here we report the incidence of post-stroke seizures in all patients diagnosed with ischemic stroke in a 3-year period. The most significant predictor was hemorrhagic transformation, and this was driven by IPH.Disclosures K. Carroll: None. E. Federico: None. I. Stafstrom: None. E. Skinner: None. M. Maraghe: None. M. E-089 Rural versus metropolitan stroke activation incidence: using Viz.ai to determine disease penetrance
ConclusionCompared to a metropolitan PSC in our network, two rural PSCs had significantly higher per capita incidence of stroke activations. There was a trend toward higher per capita thrombectomy alerts as well. Given the known health disparities in rural regions of the US, these findings provide clear confirmation of the importance of stroke care protocols in rural areas, and the value of rapid identification and triage for rural communities. Our future studies will examine more detailed patient-specific factors based on geographic region.Disclosures J. Fraser: None. J. Frank: None. A. Chen: None. M. Campbell: None. L. W...

E-132 A retrospective analysis of stroke code specificity in predicting acute ischemic events
ConclusionsThis retrospective analysis of stroke codes at MGUH during a 6-month period was 29% specific in predicting AIS. This is significantly lower than what was found in previous studies. Of stroke mimics, TME was the major etiology encompassing 43.6% of cases. The results of our study suggest additional stroke education may be needed for non-neurologists and ancillary staff to appropriately recognize stroke symptoms to better improve the utilization of hospital resources and provide appropriate and efficient care.Disclosures A. Serum amyloid A predicts poor functional outcome in patients with ischemic stroke receiving endovascular thrombectomy: a case control study
Conclusions Our findings demonstrate that plasma levels of SAA at an early stage are significant predictors for poor functional outcomes at 3 months in patients with stroke receiving EVT, indicating the substantial role of systemic inflammation in shaping stroke outcomes following EVT.

P-001 evaluation of transfer times for emergent stroke patients from regional centers to a comprehensive center
ConclusionsThe emergent transfer of stroke patients from regional medical centers to our comprehensive stroke center was extremely inefficient. The average time to transfer a patient was over an hour and a half, and some of the longest transfer times were from the nearest facilities. These findings suggest that increased efficiency in acute stroke care is needed, as clinical outcomes depend on rapid revascularization. Developing systems of care should focus on early stroke diagnosis and consideration of bypassing local hospitals to bring certain potential stroke patients directly to comprehensive centers.Abstract P-001 Fig...

P-038 Demographic disparities in proximity to certified stroke care in the United States
ConclusionsDisparities in proximity to stroke care were worse in non-urban areas than in urban areas. Non-urban census tracts with greater representation of elderly, American Indian, or uninsured persons, or low median income, were substantially more distant from certified stroke care. Quantifying disparities in proximity to timely stroke care can inform ongoing efforts for infrastructure improvements that reduce inequalities in access to care.Disclosures C. Yu: None. T.
