Watching, but not waiting: vascular neurology perspective on the disparate regulatory pathways for stroke
Vascular neurologists have keenly watched the Watchman device (Atritech, Plymouth, Minnesota, USA) regulatory approval process. We are, as always, searching for additional options in the prevention and treatment of stroke to better care for our patients, and new approaches to the management of atrial fibrillation play a large part in this effort. Recently, a Food and Drug Administration (FDA) panel voted 13:1 in favor of the Watchman device for the prevention of ischemic stroke in non-valvular atrial fibrillation.1 The panelists came to this decision after reviewing data from large randomized trials that compared anticoagu...

E-114 Rescue suction mechanical thrombectomy of medium-small vessel occlusion in a pediatric patient presented with acute ischemic stroke
ConclusionEndovascular aspiration mechanical thrombectomy of MSVO is feasible in pediatric patients presenting with AIS and associated with good functional outcome. Additionally, when acute ongoing neurological deficit is present and CTA is inconclusive, a digital subtraction angiography is deemed needed to identify MSVO as the yields of CTA diminishes significantly beyond LVO. Further studies are required.Disclosures R. Hart: None. A. Singh: None. C. Manzanero: None. Y. Lodi: None.

Response from the SAMMPRIS trial principal investigators regarding inaccuracies in this editorial
As the principal investigators for the Stenting and Aggressive Medical Management for the Prevention of Recurrent Ischemic Stroke (SAMMPRIS) trial,1 we are compelled to address a few inaccuracies regarding this study in the recent editorial by Dr Alexander.2 The first point is the patient population. The SAMMPRIS population was the right patient population to test. We had very good data from the Warfarin versus Aspirin for Symptomatic Intracranial Disease (WASID) trial that patients with recently symptomatic severe stenosis were at the highest risk for recurrent stroke.3 This high-risk population offered the best chance fo...

E-248 Postpartum vertebral artery dissections: a report of 7 cases and literature review
ConclusionA limited cohort of 33 ppVADs exist in the literature. This study contributes 7 additional cases and suggests that, despite heterogenous management of ppVAD, the resulting outcomes were favorable.Disclosures V. Lazarov: None. A. Monteiro: None. F. P-013 Use of the Solitaire Device for Emergency Revascularization of the Superior Mesenteric Artery
ConclusionsThe techniques and equipment used for acute stroke intervention may be suitable for acute recanalization of peripheral vessels, under the right circumstances. As with many surgical interventions, a multidisciplinary approach may at times yield a novel and effective strategy for a difficult clinical problem.DisclosuresJ. Dalfino: None. A. Paul: None. J. Patient selection, physician experience and antiplatelet therapy testing are critical
The response1 from the principal investigators of the Stenting and Aggressive Medical Management for the Prevention of Recurrent Ischemic Stroke (SAMMPRIS) trial2 is greatly appreciated. Healthy debate helps us weed out the details that are important in these studies and future trial design. Although the principal investigators label some of the editorial comments as ‘inaccuracies’, one could disagree based on the information listed below. Let us look at the numbers and facts. First, they state that the SAMMPRIS population was the right population to test based on the Warfarin versus Aspirin for Symptomatic Int...

Correlation between intracranial vertebral artery stenosis diameter measured by digital subtraction angiography and cross-sectional area measured by optical coherence tomography
Conclusion Measurement of the narrowest diameter was not a reliable predictor of the cross-sectional area of V4 stenosis. Larger studies are therefore needed to develop a new evaluation system based on V4 stenosis.
