Stroke vision, aphasia, neglect (VAN) assessment--a novel emergent large vessel occlusion screening tool: pilot study and comparison with current clinical severity indices
Conclusions The VAN screening tool accurately identified ELVO patients and outperformed a NIHSS ≥6 severity threshold and may best allow clinical teams to expedite care and mobilize resources for ELVO patients. A larger study to both validate this screening tool and compare with others is warranted.

Monitored anesthesia care during mechanical thrombectomy for stroke: need for data-driven and individualized decisions
Conclusions Our study showed that the overall rate of conversion from MAC to GA during MT was low (1.6%) and, while higher in posterior circulation strokes, it was not predicted by either hemispheric dominance or stroke severity. Caution should be given before changing clinical practice during moments of crisis.

P013/117 A case of delayed post-hypoxic leukoencephalopathy after ischemic stroke
ConclusionThis case illustrates that the manifestation of clinical symptoms and morphological alterations observed on MRI, indicative of DPHL, may not exclusively result from global hypoxia. Rather, such changes can also manifest in patients with a large vessel occlusion within the corresponding vascular territories.Disclosure of InterestNothing to disclose

Performance of the vision, aphasia, neglect (VAN) assessment within a single large EMS system
Conclusions Among suspected stroke patients, we found modest sensitivity and specificity of VAN to detect LVO for both EMS and ED providers. Moreover, the low PPV in our study suggests a significant number of patients with non-LVO ischemic stroke or ICH could be over-triaged with VAN.

E-058 Pilot Study: Feasibility of the Use of Ventura Emergency Large Vessel Occlusion Scale to Identify Large Vessel Occlusion Stroke
ObjectivesTo evaluate the feasibility and reliability of a self-designed Ventura Emergency Large Vessel Occlusion (ELVO) Scale performed by Emergency Medical Services (EMS). To identify, if field identification of ELVO, reduces door to groin time.BackgroundEndovascular treatment of stroke is now the standard of care for large vessel occlusion (LVO) stroke within 6 hours of onset. The outcomes of intra-arterial (IA) endovascular treatment are time dependent. The time from arrival to groin vary during the day time versus after hours or infield transfers compared to hospital to hospital transfers. Pre-identification of LVO fr...

Too good to intervene? Thrombectomy for large vessel occlusion strokes with minimal symptoms: an intention-to-treat analysis
Conclusions Thrombectomy led to a shift towards a lower NIHSS in patients with LVO presenting with minimal stroke symptoms. Despite the overall perception that this condition is benign, nearly a quarter of patients primarily treated with medical therapy did not achieve independence at 90 days.

E-125 Gaze weakness neglect and speech (GWNS): an acute ischemic stroke scale of large vessel occlusion (LVO) in the emergency department for faster treatment
ConclusionsGWNS is an AISS of LVO which could be performed rapidly in ER. GWNS score 3 and 4 are associated with LVO, especially when gage deviation/preference are present. Additionally, GWNS scale was superior to NIHSS to detect LVO. Therefore, patients with AIS with GWNS score 3 or 4 could be directly send for early MT which will save time and millions of brain cells. Further studies are needed and we are moving to direct GWNS to thrombectomy without CTA in our next phase.Disclosures Y. Lodi: None. R. Pande: None. I. Khan: None.

E-235 Automated quantification of arcuate, superior longitudinal, and inferior longitudinal fasciculi using diffusion tensor tractography correlates with speech function in post-thrombectomy stroke patients
ConclusionFor left-sided MCA occlusions, TICI scores were well correlated with increased arcuate and superior longitudinal fasciculi tract counts after MT. After recanalization of the left MCA, increased number of arcuate, superior longitudinal, and inferior longitudinal fasciculi tracts is associated with improved speech function measured by the WAB test. These two findings were not apparent in right-sided MCA occlusions, perhaps due to the minimal role of the right cerebral hemisphere in speech. Tract count analysis of the arcuate, superior longitudinal, and inferior longitudinal fasciculi for either right- or left-MCA o...

E-025 First Reported Case of Mechanical Thrombectomy for Acute Ischemic Stroke in an Individual with a Total Artificial Heart
ConclusionEVT has been proven to be safe and efficacious for AIS. Prior reports have demonstrated benefit for patients with concurrent mechanical circulatory support devices; however, our report is the first case demonstrating efficacy with a TAH device. Presence of mechanical circulatory support devices should not exclude EVT in individuals with AIS.DisclosuresM. Nezhad: None. P. Eboli: None. M. Austin: None. K. Schlick: None. M. Alexander: None.

E-057 Prospective prehospital evaluation of VAN and RACE large vessel occlusion prediction instruments on a mobile stroke unit
ConclusionsBoth VAN and RACE demonstrated measurable but limited benefit in predicting LVO in the field. In patients with LKW times of ≤1.5 hours, RACE showed a high NPV of 0.96. In this subset of patients, RACE could be used to rule out LVO in the prehospital setting, avoiding delays in care due to unnecessary transport to a thrombectomy-capable facility.Disclosures J. O-006 Van elvo (vision, aphasia, neglect) emergent large vessel occlusion study
Conclusion In this large cohort, the VAN screening tool has been validated and identified LVO more accurately than any other screening tool. This suggests that severity scoring is less accurate than the type of stroke screening. A larger study to reproduce validation in the ED and in the field for triage is warranted. Disclosures M.
